Facet Injections

The facet joints, aka zygapophyseal joints, are the joints on each side of your spine that allow motion of the spine. When these become irritated they can become a source of both local and referred pain. Facet joint injections can be injected with anesthetic (to diagnose the source of pain), corticosteroids (to reduce inflammation), or orthobiologics such as PRP or cell therapy (to help restore the normal function & stability of the joint).

At Motion is Medicine Dr. Clearfield can inject the facet joints of the cervical (neck), thoracic (upper back), and lumbar (low back) safely under ultrasound guidance(so NO RADIATION!).

Caudal Epidural Steroid Injections (ESI)

A caudal epidural steroid injection (caudal ESI) is an injection into the lowest part of the epidural space of the spine near the sacrum.

Why is it used?

A caudal ESI allows for both anesthetic and steroid to be placed directly into the epidural space to reduce chronic nerve inflammation and pain coming from the spine. Anesthetic is used to provide short term pain relief until the steroid is able to take effect (usually within 2-7 days)…

Sacroiliac Joint Injections

The sacroiliac joints (SI) are found at the bottom of the spine and are an often missed source of low back pain. SI joint and ligament pain can be debilitating, but fortunately we have a solution for this! Through injections of corticosteroids we can calm the area down allowing you to rehab it, and through Regenerative Orthopedic Medicine procedures of prolotherapy, PRP, or cell therapy we can stabilize the area and lessen the chance of recurrence.


Spinal Regenerative Orthopedic Medicine

For cases where there is a recurrence of pain, ie: “gets better, gets worse, gets better, gets worse again,” very often there is instability of the spine’s supporting structures (ligaments, tendons, and small stabilizing muscles) that leads to this instability. Regenerative Orthopedic Medicine uses the body’s natural healing abilities to strengthen and stabilize these weak and unstable structures, leading to strength, stability, and NO MORE PAIN!


Retrolaminar Cervical Nerve Block

  • Cervical radiculopathy is a painful and debilitating condition where the nerve root gets pinched or irritated by a bulging disc or arthritic facet.
  • This can result in nerve pain that will shoot down the arm and/or upper back, and can also lead to weakness of the arm, upper back, or neck.
  • When conservative options fail to relieve these symptoms, patients are often sent to pain management for cervical epidural steroid injections (ESI), often performed in a hospital or surgical center.
  • Retrolaminar Cervical Nerve Block (RCNB) is a newer approach to treat cervical radiculopathy, and can be done safer and more cost effective than cervical ESIs.
  • Ultrasound guidance is used to target the structures, and this procedure can be safely performed in our officewithout needing to put the patient under anesthesia or expose to radiation.

Thoracic Paravertebral Block

  • The Thoracic ParaVertebral Block (TPVB) is a procedure that can be performed to provide anesthesia as well as for treating acute and chronic pain of unilateral origins of the chest and abdomen.
  • The classic case where this is performed would be to alleviate the pain from shingles (aka: post-herpetic neuralgia) that has not responded to initial therapy.
  • Ultrasound guidance is used to target the structures, and this procedure can be safely performed in our officewithout needing to put the patient under anesthesia or expose to radiation.

The ribs and their related structures can be a huge source of acute and chronic pain. Whether it starts after a trauma, sleeping the wrong way or even coughing too hard, rib pain can dramatically affect the quality of your life.

At Motion is Medicine we have various treatments that can be used to treat acute and chronic rib pain:

  • Osteopathic Manipulative Treatment
    • Osteopathic Manipulative Treatment (OMT)is a hands-on treatment that allows the body to heal itself. By performing gentle manipulation Dr. Clearfield is able to guide the body back to its proper alignment and facilitate the healing process.
  • Intercostal nerve blocks
    • Intercostal nerve blocks are treatments used for typically acute and debilitating nerve pain.
    • Using ultrasound guidance, the nerves in between the levels of the ribs are identified and safely injected to calm down the pain.
  • Costotransverse Joint Injections
    • The costotransverse joints (CTJ) are the joints joining the ribs to the thoracic spine. Like other joints, these can become irritated and become a large source of pain.
    • CTJ injections can treat the source of the pain and facilitate the individual’s ability to rehab the area after. 
    • These are safely performed in our office under ultrasound guidance to ensure accuracy, safety, and efficacy.
